Like I always say. Getting the right answer to the wrong question is no more beneficial than getting the wrong answer to the right question. A true intake is not a form filled out by a client or patient. An intake is a one on one dialogue where the therapist asks extremely targeted, specific, pointed questions relevant to achieving the client or patient's goal(s).
That information is used to develop waking hypnotic suggestion, and to design suggestion to be used in the body of a session. The therapist becomes "an archeologist of the mind" sifting through the layers of information to separate what is truly relevant from useless data. Throw away your scripts and learn how to develop truly powerful sessions tailored to each client or patients specific needs. You'll have a greater success rate and have lots of fun doing it.
